Shift work — broadly defined as any work schedule that falls outside the standard 7 AM–6 PM window — affects an estimated 15–20% of workers in developed countries. The research on its long-term health impacts is unambiguous and, for many workers, alarming.

The Key Health Risks

Cardiovascular disease. Meta-analyses consistently show a 40% higher risk of coronary heart disease in long-term shift workers versus day workers, after controlling for other risk factors. The mechanism is well understood: circadian disruption elevates cortisol, inflammatory markers and blood pressure during hours when the body expects to be asleep.

Type 2 diabetes. Night shift workers have significantly higher rates of insulin resistance and metabolic syndrome. Eating at circadian-misaligned times — such as a full meal at 3 AM — produces a blood glucose spike roughly twice as large as the same meal eaten at noon.

Mental health. Shift workers have higher rates of depression, anxiety and burnout. Chronic sleep deprivation reduces emotional regulation capacity, increases cortisol reactivity and depletes the dopaminergic systems involved in mood.

Cancer risk. The International Agency for Research on Cancer classifies night shift work as a probable human carcinogen (Group 2A), primarily because of the suppression of melatonin — which has established anti-tumour properties — during light-at-night exposure.

Protective Strategies That Have Evidence

The good news is that risk is substantially modifiable. The most evidence-backed interventions are: maintaining the same sleep-wake schedule even on days off (reduces cardiovascular markers significantly), time-restricted eating within a 10-hour window timed to your waking hours, bright light therapy on waking (10–30 minutes of 10,000 lux equivalent), avoiding light exposure on the commute home after a night shift, and regular aerobic exercise on days off.
